Important:  Please read the following guidelines and check the box below before submitting.  (Review the Permits page for more information on procedures and permissible activities.)

To be granted a permit for work at the University of Akron Field Station, you must agree to the following terms:

• Users are responsible for obtaining the appropriate state or federal permits for the conduct of their specific activities (e.g., when working with regulated species) and must be able to provide documentation of such permits if asked.

• Users are responsible for removing all markers, etc. when the activity is completed.

• Users are responsible for cleaning up the Martin Center for Field Studies and Environmental Education if the building is used.

• Users must file an annual (due in yearly increments based on the date of the permit) and final report. Such reports will include: user days, a summary of results of the project(s), a list of data generated and contact information for those interested in the data, and a list of publications resulting from the project(s).

• Any publications resulting from research conducted at the University of Akron Field Station must acknowledge this use by referencing the project's permit number. A copy of any such publication should be filed with the Field Station committee.

• To abide by the rules and regulations of the University of Akron Field Station and the Bath Nature Preserve in any and all conduct of activities.

By checking this box, I agree to the above terms and state that all of the above information is correct to the best of my knowledge. I also agree to amend my above permit request if my plans change such that they are no longer well represented in the information supplied in this permit request. If I fail to notify the Field Station oversight committee of significant changes in my research, or if I do not follow the rules of the station or Bath Nature Preserve, I realize that my permit can be revoked, and I can lose my privileges for activities at The University of Akron Field Station and Bath Nature Preserve.
