Office of International Programs (OIP), The University of Akron, Polsky
Building Room 483:

1. Completed application form (click
here for application form)
2. One passport-size photograph (2” x 2”, head shot, facing forward,
white background, high resolution, photo paper)
3. Proof of student/teacher status (see the application form)
4. Driver’s license, state identification card, or passport
5. Check or money order made payable to The University of Akron for
$22.00

There is usually
a 48-hour turn-around time in the OIP for issuing the cards.
