FORMAT FOR VOLUNTEER PROJECT FEEDBACK

III.  Information About Agency

 

Write at least one good, informative paragraph about the agency (what it does, who it serves, where it’s located, etc.)  Include information such as background of supervisor/contact, number of years in operation, collaborative efforts with other agencies, etc.  In addition to your description, you can attach informational handouts or flyers provided by the agency if you like.  Include web page and contact information when applicable.

 

Example:

Project RISE is a collaborative effort by Akron Public Schools and local shelters to provide supplemental educational services to children and youth who are homeless.  Children are provided with tutoring and other services and resources to effectively support their education. 

Some of the services include:  tutors for children living in homeless shelters, the learning doll project, field trips with children, parent education, and referrals to relevant services and resources.  Project RISE provides services for all area shelters including Safe Landing Shelters, Battered Women’s Shelter, Haven of Rest, Access, Inc., Harvest Home, House of Peace, and the Salvation Army.

 

V.  Personal Evaluation

 

Give your own reaction to your volunteer experience.  Write a minimum of two typed, double-spaced pages for this section. Can include (but need not be limited to) things such as how rewarding you found the experience, why you think the organization is especially beneficial for the population it serves, what might be changed about the organization or its services to make it more effective, and whether or not you would like to volunteer at this agency or another like it in the future.

 

Total page requirement = 5-7 pages for all the above sections (not including attached form)

 

You must include the signed volunteer report when you turn in your written project report.
